- Confirm the selected eye and face protection matches the task hazard, including impact, splash, dust, heat, optical radiation, flying particles, or combined exposure where applicable.
- Verify side protection, seals, vents, brow protection, face coverage, filter lenses, shade selection, and compatible primary eye protection are present where the task requires them.
- Check prescription eyewear arrangements do not prevent the protective device from fitting correctly and that over-spectacle or prescription safety options remain usable.
- Confirm lenses and visors are clean enough for safe visibility and are free from fogging, chemical film, severe pitting, or damage that could distort the user's view.
- Remove eye or face protection from service when impact damage, chemical attack, heat damage, failed straps, loose lenses, damaged shields, or reduced visibility compromises protection.
Section 5Hand, arm, body, and protective clothing
- Inspect gloves, gauntlets, sleeves, aprons, coveralls, chemical suits, cut-resistant garments, heat-resistant clothing, and other protective clothing for holes, tears, cuts, abrasion, burns, swelling, stiffness, or contamination.
- Confirm glove and clothing material is suitable for the actual chemical, heat, cold, cut, abrasion, puncture, biological, electrical, or mechanical hazard being controlled.
- Verify glove size, cuff length, sleeve overlap, garment closure, body coverage, mobility, and interface with other PPE provide continuous protection during the task.
- Check chemical-resistant PPE has no permeation, degradation, swelling, cracking, delamination, discoloration, or contamination that may reduce protection.
- Confirm disposable PPE is not reused beyond its intended use and reusable protective clothing is cleaned, inspected, and stored before reissue.
- Remove torn, punctured, heavily contaminated, heat-damaged, chemically degraded, electrically compromised, or otherwise unreliable hand and body protection from service.
Section 7Hearing protection and noise-related PPE
- Inspect earmuffs, earplugs, banded plugs, communication headsets, helmet-mounted hearing protectors, and related components for damage, contamination, hardened cushions, split seals, or loss of tension.
- Confirm the selected hearing protection provides the protection required by the site's noise assessment and is suitable for the employee, task, communication needs, and other PPE worn.
- Verify earmuff cushions seal evenly around the ear and are not broken by eyewear temples, hair, helmet straps, respirator components, or other interference.
- Check reusable earplugs are clean, undamaged, correctly sized, and stored hygienically, and that disposable plugs are replaced as intended.
- Confirm employees know how to insert, fit, adjust, wear, clean, and store hearing protection and understand when protection is mandatory.
- Replace hearing protection with cracked cups, damaged bands, hardened cushions, stretched components, contaminated plugs, poor fit, or any condition that prevents a reliable seal.
Section 9Fall protection PPE and personal fall-arrest equipment
- Inspect full-body harnesses, lanyards, energy absorbers, self-retracting devices, connectors, snap hooks, carabiners, lifelines, rope grabs, and related personal fall equipment before use.
- Check webbing, stitching, rope, cables, housings, labels, D-rings, buckles, connectors, hooks, gates, energy absorbers, and attachment points for cuts, fraying, burns, corrosion, deformation, impact, or contamination.
- Confirm the selected system matches the task, anchor arrangement, fall-clearance requirement, user weight, rescue plan, and approved work-at-height method.
- Verify connectors close and lock correctly, harness adjustment provides secure fit, attachment points are used as intended, and incompatible connections are not made.
- Confirm equipment exposed to a fall event, excessive load, serious impact, chemical contamination, heat damage, or questionable service condition is immediately removed from use pending competent evaluation or disposal.
- Check periodic competent-person inspections, identification records, inspection history, retirement dates, storage, and manufacturer-required servicing are current where applicable.
Section 2General PPE condition, identity, fit, compatibility, and serviceability
- Inspect each sampled PPE item for cracks, tears, cuts, holes, deformation, corrosion, contamination, missing parts, damaged fasteners, worn straps, or other defects.
- Confirm labels, model details, size, rating, class, filter or cartridge type, serial number, user identification, and other required markings remain legible where applicable.
- Verify the PPE fits the assigned user correctly and can be adjusted without excessive looseness, pressure points, slipping, restricted movement, or loss of protection.
- Check multiple PPE items can be worn together without interfering with seal, fit, visibility, hearing protection, helmet position, fall protection, or safe task performance.
- Confirm defective, damaged, contaminated, expired, altered, or questionable PPE is removed from service and clearly segregated from usable equipment.
- Record item type, user or location, defect, immediate control, replacement need, owner, due date, and evidence required before the PPE is returned to service.
Section 4Head protection, helmets, bump caps, and accessories
- Inspect safety helmets, hard hats, bump caps, shells, suspension systems, chin straps, sweatbands, and accessories for cracks, dents, punctures, brittleness, cuts, deformation, or missing parts.
- Confirm the head protection type and rating are suitable for the identified impact, penetration, falling-object, or electrical hazard where applicable.
- Check suspension, harness, adjustment, chin strap, and retention components are correctly fitted and hold the helmet securely without unauthorized modification.
- Verify paint, stickers, drilling, heating, chemical exposure, incompatible accessories, or other alterations have not damaged or weakened the protective shell.
- Confirm helmet accessories such as face shields, hearing protection, lights, visors, or winter liners are approved for use with the helmet and do not weaken fit or protection.
- Remove helmets from service after significant impact, visible damage, chemical or heat degradation, failed suspension, or when replacement criteria in the manufacturer or site standard are met.
Section 6Foot, leg, high-visibility, and task-specific protective wear
- Inspect safety footwear, boots, metatarsal protection, toe protection, puncture-resistant soles, gaiters, leggings, and overshoes for cuts, sole separation, worn tread, exposed protective parts, or contamination.
- Confirm footwear and leg protection match the task hazards such as impact, compression, puncture, slip, chemical exposure, heat, wet conditions, or electrical risk where applicable.
- Verify soles provide adequate grip for the work surface and that laces, fasteners, zippers, straps, and closures secure the footwear without creating trip hazards.
- Inspect high-visibility vests, jackets, trousers, armbands, and reflective materials for fading, contamination, tears, missing closures, or reduced visibility.
- Confirm specialist protective wear such as arc-rated clothing, chainsaw protection, molten-metal protection, cooling garments, or other task-specific PPE is correct for the approved task and remains serviceable.
- Remove protective footwear or clothing from service when structural damage, contamination, worn protective components, loss of visibility, or other defects reduce required protection.
Section 8Respiratory protective equipment
- Confirm respirators are selected under the approved respiratory-protection program for the identified contaminant, concentration, oxygen condition, task, and work environment.
- Inspect facepieces, straps, valves, seals, cartridges, filters, hoses, regulators, batteries, breathing tubes, connectors, and housings for damage, contamination, blockage, wear, or missing parts.
- Verify required fit testing, medical clearance, user training, and other prerequisites are current for employees using tight-fitting or specialist respirators where applicable.
- Confirm users perform the required pre-use checks and seal checks for tight-fitting respirators and that facial hair, eyewear, scars, headgear, or other interference does not break the seal.
- Check cartridges, filters, canisters, batteries, cylinders, or other consumables are the correct type, within approved service life, stored correctly, and changed under the site's replacement schedule.
- Remove respirators from service when seals, straps, valves, filters, regulators, hoses, batteries, facepieces, or any critical component cannot provide reliable protection.
Section 10Cleaning, storage, replacement, training, records, and corrective action
- Verify reusable PPE is cleaned and disinfected using methods that do not damage protective materials, coatings, seals, straps, filters, or electrical properties.
- Confirm PPE is stored clean, dry, protected from sunlight, heat, moisture, chemicals, crushing, deformation, contamination, and unauthorized use.
- Check replacement stock and consumables are available for critical PPE so damaged, contaminated, expired, or lost equipment can be replaced without unsafe delay.
- Verify employees and contractors have received required PPE training covering why PPE is needed, correct use, limitations, fitting, inspection, care, storage, and replacement.
