Section 1Program setup, respiratory hazards, scope, and administration
- Confirm the project, work area, task, inspection date, contractor, respirator users, supervisor, program administrator, inspector, and approver.
- Identify the airborne contaminants, dusts, fumes, mists, gases, vapors, oxygen-deficient conditions, emergency scenarios, and work activities included in the review.
- Verify a written respiratory protection program is established where respirator use is required and includes worksite-specific procedures for selection, medical evaluation, fit testing, use, maintenance, training, and program evaluation.
- Confirm the respiratory protection program is administered by a suitably qualified program administrator and is updated when workplace conditions or respirator use changes.
- Review previous exposure findings, respirator failures, fit-test issues, cartridge breakthrough concerns, employee complaints, medical restrictions, and overdue corrective actions.
- Capture the inspection start time, work location, and approved reference photos showing representative respiratory hazards, respirators, storage, and work controls.
Section 3Medical evaluation and employee ability to use a respirator
- Verify each employee required to use a respirator has received a medical evaluation before fit testing or required workplace use.
- Confirm the medical evaluation is performed by a physician or other licensed health care professional using the required questionnaire or an examination that obtains equivalent information.
- Verify employees can complete the medical evaluation confidentially during normal working hours or at a reasonable time and can communicate with the evaluating health care professional.
- Confirm the employer receives only the required written recommendation regarding the employee's ability to use a respirator, any limitations, and any need for follow-up evaluation.
- Check additional medical evaluation is provided when an employee reports relevant symptoms, a health care professional or program administrator recommends reevaluation, fit testing or program evaluation indicates a need, or workplace demands substantially change.
- Prevent required respirator use when medical clearance is missing, expired under site rules, restricted, or otherwise not sufficient for the employee and assigned respirator.
Section 5Routine respirator use, seal integrity, work conditions, and continuing effectiveness
- Observe respirator donning and use to confirm the facepiece is correctly positioned, straps are adjusted, valves and components are assembled, and the respirator is worn for the full exposure period.
- Verify corrective eyewear, goggles, face shields, helmets, hearing protection, clothing, or other PPE do not interfere with the respirator seal or function.
- Confirm appropriate surveillance of work-area conditions and employee exposure or stress is maintained while respirators are being used.
- Require workers to leave the respirator-use area if they detect gas or vapor breakthrough, changes in breathing resistance, facepiece leakage, eye or skin irritation requiring attention, or the need to replace cartridges, filters, or canisters.
- Verify defective or leaking respirators are repaired or replaced before the employee is allowed to return to the respirator-use area.
- Reevaluate respirator effectiveness whenever workplace conditions, contaminant concentration, work rate, temperature, humidity, or employee stress changes in a way that could affect protection.
Section 7Cleaning, disinfecting, storage, and contamination control
- Verify each respirator provided to an employee is clean, sanitary, and in good working order before use.
- Confirm respirators issued exclusively to one employee are cleaned and disinfected as often as necessary to maintain a sanitary condition.
- Verify respirators used by more than one employee are cleaned and disinfected before being worn by different individuals.
- Confirm emergency-use respirators and respirators used in fit testing or training are cleaned and disinfected at the required intervals.
- Inspect respirator storage to ensure equipment is protected from damage, contamination, dust, sunlight, extreme temperatures, excessive moisture, and damaging chemicals.
- Verify respirators are packed or stored to prevent deformation of the facepiece and valves and that emergency respirators remain clearly identified and readily accessible where required.
Section 9IDLH, unknown atmospheres, supplied air, standby personnel, and emergency response
- Treat atmospheres that are immediately dangerous to life or health, oxygen-deficient beyond permitted limits, or insufficiently characterized as requiring the appropriate IDLH respiratory protection strategy.
- Verify workers entering IDLH atmospheres use a full-facepiece pressure-demand self-contained breathing apparatus with the required service duration or an approved pressure-demand supplied-air respirator with auxiliary self-contained air supply.
- Confirm at least one employee, or more when needed, remains outside the IDLH atmosphere and maintains visual, voice, or signal-line communication with workers inside.
- Verify outside standby personnel are trained and equipped to provide effective emergency rescue and that required employer or designee notification and assistance arrangements are in place.
- Check retrieval, rescue, supplied-air hose protection, emergency escape air, communication, access, and casualty-removal arrangements against the actual work area and foreseeable emergency.
- Stop entry immediately when standby coverage, communications, breathing-air supply, escape capacity, rescue readiness, or the required IDLH respirator configuration is not fully available.
Section 2Exposure assessment, respirator selection, certification, and protection level
- Verify the employer has evaluated the respiratory hazards and estimated employee exposure well enough to select respirators appropriate for the actual workplace conditions.
- Confirm the selected respirator type is suitable for the contaminant, concentration, physical form, oxygen level, task duration, work rate, temperature, humidity, and foreseeable emergency conditions.
- Verify respirators, cartridges, filters, canisters, and components are NIOSH-certified and are used in accordance with the conditions of their certification.
- Confirm the respirator provides an assigned protection factor and maximum use concentration appropriate for the exposure and selected facepiece or respirator class.
- Check particulate, gas, vapor, combination, powered air-purifying, supplied-air, or self-contained respirators are selected according to the hazard and any substance-specific requirements that apply.
- Stop the task where the hazard cannot be adequately characterized or the selected respirator cannot be shown to provide sufficient protection for the exposure.
Section 4Fit testing, facepiece selection, facial hair, and user seal checks
- Verify every employee using a tight-fitting respirator has passed an appropriate qualitative or quantitative fit test for the exact make, model, style, and size used.
- Confirm fit testing is completed before initial use, whenever a different respirator facepiece is used, and at least annually thereafter.
- Check additional fit testing is completed when physical changes such as facial scarring, dental changes, cosmetic surgery, or obvious body-weight change could affect the facepiece seal.
- Verify employees are not permitted to wear tight-fitting respirators when facial hair, headgear, eyewear, or another condition interferes with the sealing surface or valve function.
- Confirm the worker performs a user seal check every time a tight-fitting respirator is put on, using the approved positive/negative pressure method or equivalent manufacturer method.
- Verify workers understand that a user seal check does not replace the required qualitative or quantitative fit test.
Section 6Filters, cartridges, canisters, change schedules, and particulate loading
- Verify each filter, cartridge, and canister is the correct NIOSH-approved type for the contaminant and respirator and that approval labels or identification remain legible.
- Confirm gas and vapor cartridges use an approved end-of-service-life indicator or a documented cartridge change schedule based on objective information or data.
- Verify cartridge change schedules account for exposure concentration, temperature, humidity, breathing rate, work rate, interfering chemicals, and other factors that can reduce service life.
- Confirm odor, taste, or other warning properties are not being used as the primary basis for determining gas or vapor cartridge service life.
- Inspect particulate filters for damage, loading, excessive breathing resistance, moisture, contamination, improper installation, or replacement conditions defined by the manufacturer or program.
- Remove filters, cartridges, or canisters from service when they are damaged, contaminated, expired, outside the approved change schedule, or otherwise unable to provide reliable protection.
Section 8Inspection, maintenance, repair, emergency respirators, and breathing air
- Verify respirators used in routine situations are inspected before each use and during cleaning for function, tightness of connections, and condition of critical components.
- Inspect facepieces, straps, valves, connecting tubes, cartridges, canisters, filters, elastomeric parts, regulators, hoses, batteries, cylinders, and alarms as applicable.
- Confirm emergency-use respirators are inspected at least monthly, according to manufacturer recommendations, and checked for proper function before and after each use.
- Verify self-contained breathing apparatus cylinders are maintained at the required charge level and that regulators and warning devices function properly.
- Confirm respirators that fail inspection are removed from service and repaired only by appropriately trained persons using manufacturer-approved parts and procedures.
- Where atmosphere-supplying respirators are used, verify breathing-air quality, quantity, flow, compressor or cylinder controls, couplings, and contamination safeguards meet the applicable respiratory protection program requirements.
Section 10Training, program evaluation, records, corrective actions, and sign-off
- Verify respirator users understand why respiratory protection is necessary and how improper fit, use, or maintenance can compromise protection.
- Confirm training covers respirator limitations and capabilities, emergency use, inspection, donning and removal, seal checks, maintenance, storage, and recognition of signs or symptoms that may affect safe respirator use.
- Verify required training is completed before initial respirator use and retraining is provided annually and when workplace changes, respirator changes, or user knowledge or performance indicates a need.
- Confirm the employer periodically evaluates the workplace and consults respirator users as necessary to verify the respiratory protection program is being properly implemented and remains effective.
- Assign each respiratory protection finding an owner, priority, due date, immediate restriction where required, corrective action, retraining or refit need, and closure evidence requirement.
- Record the final respiratory protection review result, critical restrictions, respirators removed from service, medical or fit-test follow-up, next review date, program administrator, inspector, supervisor, approver, date, time, and sign-off.

How often is respirator fit testing required?+
For tight-fitting respirators covered by OSHA's respiratory protection standard, fit testing is required before initial use, whenever a different facepiece is used, and at least annually. Additional fit testing is required when physical changes could affect the facepiece seal.
Is a user seal check the same as a fit test?+
No. A user seal check is performed every time a tight-fitting respirator is put on to check whether an adequate seal has been achieved. It does not replace the required qualitative or quantitative fit test.
When must a worker receive a medical evaluation for respirator use?+
OSHA requires the medical evaluation before the employee is fit tested or required to use a respirator in the workplace. Additional evaluation can be required when symptoms, program findings, health-care recommendations, or changes in work demands indicate a need.
How should gas and vapor cartridge replacement be controlled?+
Use an approved end-of-service-life indicator or a documented change schedule based on objective information or data. Odor or taste should not be used as the primary method for deciding when a gas or vapor cartridge has reached the end of its service life.
