  • Apply immediate controls such as closing doors, restoring refrigeration, stopping loading, holding dispatch, or moving stock when safe and appropriate.
  • Record the initial status as contained, ongoing, under investigation, emergency escalation required, or specialist support required.

  • Review logger, sensor, chart, alarm, manual-reading, vehicle, or other records to determine when the temperature first moved outside the approved range.
  • Record the peak high or low temperature reached during the known excursion period where reliable evidence is available.
  • Estimate or confirm excursion duration using the most reliable time-stamped data available.
  • Check whether the excursion was continuous, intermittent, repeated, or associated with recovery cycles or door events.
  • Compare manual and automated readings where available to identify sensor disagreement or measurement uncertainty.
  • Document gaps, missing data, device failure, uncertain start time, or other limitations that affect confidence in the excursion profile.

  • Confirm the product's approved storage or transport range and any permitted excursion or stability information used for assessment.
  • Review product temperature, packaging, product state, visible condition, freeze or heat exposure, and other available evidence where relevant.
  • Check receiving, staging, loading, transport, transfer, or dispatch records for additional exposure outside the primary monitored location.
  • Consider whether only part of the inventory may have been affected because of location, airflow, pallet position, load pattern, or timing.
  • Route the assessment to the authorized quality, product, customer, supplier, or technical owner where required by the programme.
  • Keep inventory blocked when product impact cannot be confidently assessed using approved evidence and criteria.
Section 7Disposition, release, rejection, and customer action
  • Record the authorized disposition decision for affected inventory, such as release, conditional release, transfer, rework, return, rejection, destruction, or other approved outcome.
  • Confirm the disposition decision is supported by the product assessment, excursion profile, quality review, customer rule, or other controlling evidence.
  • Update physical labels, WMS or ERP status, hold codes, quarantine records, and inventory locations to match the approved disposition.
  • Document any customer, supplier, carrier, quality, regulatory, or management notification required by the approved programme.
  • Verify rejected, destroyed, returned, or otherwise non-released stock cannot re-enter normal inventory without controlled authorization.
  • Do not release affected inventory until the authorized disposition and required evidence are complete.
Section 9Root cause, trend review, and preventive action
  • Classify the root cause by refrigeration, power, door management, loading, staging, airflow, sensor, alarm, communication, procedure, training, or other approved category.
  • Review whether similar excursions have occurred in the same room, vehicle, route, product, shift, equipment, or time period.
  • Check response time, alarm acknowledgement, stock identification, escalation, maintenance, and disposition steps for delays or control gaps.
  • Create preventive actions for recurring equipment faults, door discipline, backup power, monitoring gaps, training, loading practices, or capacity issues.
  • Update SOPs, alarm thresholds, escalation contacts, maintenance plans, monitoring locations, training, or contingency arrangements where justified.
  • Share significant recurring trends with the responsible operations, quality, maintenance, cold-chain, transport, or management owner.
Section 2Affected stock, lots, and inventory hold
  • Identify all potentially affected SKUs, lots or batches, serials, pallets, handling units, quantities, locations, shipments, or customers where traceability requires it.
  • Place affected inventory on the approved physical and system hold, quarantine, blocked, or investigation status.
  • Prevent picking, dispatch, transfer, use, or release of affected inventory until authorized disposition is complete.
  • Separate clearly affected stock from unaffected inventory where practical and maintain traceable labels and handling-unit IDs.
  • Check transferred, staged, loaded, delivered, or returned inventory that may have shared the same excursion window or equipment condition.
  • Escalate immediately when affected inventory cannot be identified or traceability is insufficient to define the exposure scope.
Section 4Equipment, power, and environmental investigation
  • Inspect refrigeration, freezer, chiller, HVAC, vehicle unit, controller, compressor, evaporator, condenser, fan, or other relevant equipment for active faults.
  • Check doors, curtains, seals, gaskets, latches, loading practices, defrost status, airflow, icing, blocked vents, or excessive door-open time.
  • Review power loss, electrical faults, breaker trips, generator issues, voltage events, maintenance work, shutdowns, or control resets around the excursion period.
  • Inspect sensors, probes, loggers, alarms, wiring, communication links, batteries, or device placement for measurement or monitoring faults.
  • Check ambient conditions, weather, staging exposure, loading congestion, product load, or unusual operating activity that may have contributed.
  • Record the most likely equipment, environment, process, monitoring, or handling causes requiring further investigation.
Section 6Maintenance and process corrective actions
  • Create maintenance action for refrigeration, sensors, alarms, power systems, doors, seals, controls, or other failed equipment identified during the investigation.
  • Record temporary controls used until permanent repair is complete, such as alternate storage, backup monitoring, reduced door opening, or additional checks.
  • Correct process causes such as poor staging discipline, prolonged loading, blocked airflow, incorrect setpoint, door management, or missed alarm response.
  • Check corrective work includes the responsible owner, due date, service record, parts or repair reference, and evidence needed for verification.
  • Escalate repeated failures, delayed repairs, recurring alarm issues, or temporary controls that remain in place longer than approved.
  • Verify critical equipment repairs or process changes are complete before returning the affected area or vehicle to normal service where required.
Section 8Monitoring recovery and requalification
  • Confirm the storage area, vehicle, equipment, or process has returned to the approved temperature range after corrective action.
  • Check temperature remains stable for the required recovery or observation period before normal operation resumes where applicable.
  • Verify replacement, repaired, or suspect sensors and thermometers are checked or calibrated as required before being relied upon again.
  • Confirm alarm functions, communication, data logging, setpoints, and notification routes operate correctly after repair or reset.
  • Check product, ambient, or independent verification readings agree reasonably with the primary monitoring system where required.
  • Keep additional monitoring in place when the area or equipment has not yet demonstrated stable control.
Section 10Evidence, verification, and final closure
  • Compile the excursion record with timing, temperature profile, affected stock, logger or sensor data, photographs, maintenance evidence, investigation, and disposition.
  • Confirm corrective and preventive actions have named owners, due dates, evidence expectations, and closure criteria.
  • Verify the affected area, vehicle, device, or process is operating acceptably before all temporary controls are removed.
  • Check blocked or held inventory has a final documented disposition and system status before the event is closed.
  • Record any overdue actions, monitoring follow-up, next review date, responsible cold-chain owner, quality owner where applicable, and management escalation.
  • Record final excursion status as closed, monitoring ongoing, corrective action open, disposition pending, or further investigation required.
