Section 1Hazard assessment, task mapping, and protection requirements
- Confirm a current PPE hazard assessment identifies tasks that can expose eyes, face, hands, or arms to impact, particles, chemicals, heat, cuts, punctures, abrasion, or other injury.
- Verify the assessment distinguishes hazards that require eye protection, face protection in addition to eye protection, hand protection, arm protection, or task-specific combinations.
- Confirm engineering, guarding, containment, ventilation, work-practice, and administrative controls are considered before relying on PPE for the remaining exposure.
- Verify required protection is defined by task or area and communicated through SOPs, signage, permits, work instructions, or other controlled methods.
- Confirm selected protectors are compatible with other required PPE such as respirators, helmets, hearing protection, welding equipment, or protective clothing.
- Review incidents, near misses, process changes, new chemicals, tools, machines, and employee feedback for updates to the hand-and-eye protection assessment.
Section 3Chemical splash, dust, vapor, and face protection controls
- Confirm tasks involving corrosive liquids, hazardous chemicals, irritating dusts, gases, vapors, or splash risk use suitable eye or face protection.
- Verify chemical-splash goggles provide the required seal and coverage and are not replaced by open safety spectacles when splash protection is needed.
- Where face shields are required, confirm they are used with appropriate primary eye protection when the hazard assessment requires protection beneath the shield.
- Check face shields, goggles, vents, seals, straps, and lenses are compatible with the chemical, temperature, and duration of the task.
- Confirm employees can safely remove contaminated eye or face protection without transferring hazardous material to the eyes, skin, or clean surfaces.
- Verify contaminated reusable eye and face protection is decontaminated, inspected, and stored separately from clean equipment before reuse.
Section 5Eye-protector fit, prescription use, cleaning, and storage
- Confirm eye and face protectors fit securely, remain in position during the task, and do not create pressure points, blind spots, or unsafe interference.
- Verify employees who require prescription lenses use protective eyewear that incorporates the prescription or can be worn over corrective lenses without disturbing either position.
- Check employees have access to suitable anti-fog, ventilation, or task controls where fogging can cause the protector to be removed or impair safe vision.
- Verify shared eye and face protection is cleaned and disinfected between users where required by site hygiene controls.
- Confirm lenses are cleaned using methods and materials that do not damage coatings, optical quality, seals, or protective performance.
- Inspect storage locations and cases to ensure eye and face protection is kept clean, dry, protected from scratching, deformation, chemical exposure, and uncontrolled sunlight or heat.
Section 7Cut, puncture, abrasion, impact, and mechanical hand protection
- Confirm cut-resistant hand protection is selected for tasks involving sharp edges, blades, sheet metal, glass, wire, or other laceration hazards.
- Verify puncture-resistant protection is used where nails, needles, wire ends, splinters, swarf, or pointed components can penetrate ordinary gloves.
- Check abrasion-resistant gloves are appropriate for repeated handling of rough, abrasive, or friction-producing materials.
- Where crushing or impact to the hand is credible, verify gloves or engineered controls address the specific impact hazard without encouraging hand placement in pinch points.
- Confirm gloves are not worn around exposed rotating parts or entanglement hazards when glove use would increase the risk, unless the task has a specifically approved control method.
- Inspect mechanical-protection gloves for cuts, worn coatings, broken seams, exposed liners, embedded sharp objects, excessive contamination, or loss of grip.
Section 9Inspection, use, hygiene, change-out, and storage of hand protection
- Confirm employees inspect reusable gloves and arm protection before use for tears, holes, pinholes, seam failure, hardening, swelling, contamination, or other damage.
- Verify single-use gloves are not washed, reused, or worn beyond their intended service condition when reuse would reduce protection or hygiene.
- Check donning and doffing practices prevent contamination of bare skin, clothing, controls, tools, clean products, and work surfaces.
- Confirm gloves are changed between incompatible tasks, chemicals, contamination zones, or product-contact activities where cross-contamination is a hazard.
- Verify wet, contaminated, or damaged reusable gloves are segregated from clean stock and are not returned to service until properly treated and inspected.
- Inspect glove storage for protection from chemicals, oils, ozone, sunlight, heat, moisture, sharp objects, crushing, and uncontrolled mixing of clean and used equipment.
Section 2Impact, flying-particle, and projectile eye protection
- Confirm employees exposed to flying particles, fragments, chips, dust, or projectiles wear eye protection selected for the impact hazard.
- Verify eye protection used for flying-object hazards provides suitable side protection, including permanent or compliant detachable side protectors where applicable.
- Check grinding, cutting, chipping, drilling, machining, and similar tasks use the level of eye and face protection defined by the hazard assessment.
- Confirm nearby employees and passers-by are protected from secondary exposure created by sparks, fragments, ricochet, or ejected material.
- Inspect safety spectacles, goggles, side shields, lenses, frames, and retainers for cracks, deep scratches, looseness, distortion, or impact damage.
- Verify employees do not substitute ordinary prescription, fashion, or non-rated eyewear for required occupational eye protection.
Section 4Optical radiation, welding, cutting, and hot-work eye protection
- Confirm employees exposed to potentially injurious light radiation use eye or face protection with filter lenses appropriate to the operation.
- Verify welding helmets, hand shields, goggles, and filter lenses are selected for the process and minimum protective shade required by the applicable standard or approved procedure.
- Check helmets and shields provide adequate coverage from sparks, spatter, radiant energy, and nearby operations without restricting safe movement or visibility.
- Confirm helpers, observers, and nearby employees are protected from arc flash, ultraviolet, infrared, or intense visible-light exposure through suitable barriers or PPE.
- Inspect filter lenses, cover plates, auto-darkening equipment, sensors, headgear, and shells for cracks, contamination, delayed operation, or other defects.
- Verify damaged or questionable welding eye/face protection is removed from service and replacement components match the approved equipment configuration.
Section 6Hand-hazard assessment and glove selection
- Confirm employees use appropriate hand protection where hands are exposed to harmful skin absorption, severe cuts, lacerations, abrasions, punctures, burns, or harmful temperature extremes.
- Verify glove selection considers the task, material handled, type of hazard, exposure duration, conditions of use, fit, dexterity, and manufacturer limitations.
- Confirm glove specifications or selection charts are available for high-risk tasks and are reviewed when materials, chemicals, or processes change.
- Verify gloves selected for one hazard do not create an unacceptable new hazard such as entanglement, reduced grip, loss of tactile control, or chemical incompatibility.
- Check sleeve, gauntlet, cuff, or arm protection is added where the hazard extends beyond the hands or where splash and sharp-edge exposure can reach the wrist or forearm.
- Confirm glove sizes and styles are available so employees can obtain secure fit without excessive looseness, compression, or loss of dexterity.
Section 8Chemical, thermal, hot-surface, and cold hand protection
- Confirm chemical-resistant gloves are selected for the specific substance or mixture and the expected splash, intermittent-contact, or immersion condition.
- Verify glove material, thickness, degradation, permeation, breakthrough, and change-out information are available for significant chemical exposures.
- Check chemical gloves are replaced when contaminated, swollen, softened, cracked, discolored, sticky, stiffened, or otherwise degraded beyond safe-use criteria.
- Confirm heat-resistant gloves or mitts are used for hot surfaces, molten or heated materials, steam, radiant heat, or other thermal-burn hazards where required.
- Verify cold-protection gloves maintain insulation, fit, grip, and dexterity for refrigerated, cryogenic, outdoor, or other harmful cold exposures.
- Confirm reusable chemical or thermal gloves are cleaned, dried, decontaminated, inspected, and stored according to manufacturer and site requirements before reuse.
Section 10Training, compliance observation, incidents, corrective action, and sign-off
- Confirm affected employees are trained on when hand, eye, and face protection is necessary; what protection is required; how to wear it; limitations; care; useful life; and disposal.
- Verify employees demonstrate correct selection, fit, use, removal, inspection, and storage of the protection required for their tasks.
- Review eye injuries, hand injuries, first-aid cases, near misses, PPE failures, repeated replacements, and non-use observations for recurring causes and control gaps.
- Assign each failed inspection item to a named owner with priority, due date, temporary control, required evidence, and escalation route where necessary.
- Verify corrective-action closure through repeat observation, replacement evidence, updated assessment or SOP, retraining, and reviewer approval.
- Record the final inspection result, unresolved critical risks, restricted work if applicable, next review date, inspector, reviewer, and management sign-off.

Selection should be based on the hazards identified, the task performed, conditions present, duration of use, and the performance and limitations of the available glove materials. Chemical compatibility and change-out information are especially important for chemical exposure.
Can a face shield replace safety glasses or goggles?
A face shield protects a larger area of the face but may not provide the same primary eye protection as spectacles or goggles. Use the combination defined by the hazard assessment and applicable requirements for the specific task.
