Section 1Forklift details, task, and operator authorization
  • Confirm site, work area, forklift type, asset or fleet ID, inspection date, shift, and pre-use inspection type are recorded.
  • Record manufacturer or model where used, hour-meter reading where applicable, current attachment, and planned operating zone.
  • Confirm the operator is trained, evaluated, and authorized for the specific forklift type and attachment they will use.
  • Verify the forklift is suitable for the planned load, rated capacity, aisle, rack height, dock, trailer, floor, and environmental condition.
  • Review previous reported defects, recent impacts, maintenance work, breakdowns, battery or fuel issues, and unresolved repair items.
  • Record the initial forklift status as serviceable, restricted, repair before use, or out of service.
Section 3Tires, wheels, undercarriage, and visible leaks
  • Inspect drive, steer, load, cushion, solid, pneumatic, polyurethane, or caster wheels for cuts, chunking, flat spots, embedded objects, or severe wear.
  • Check pneumatic tire inflation where applicable against the approved fleet or manufacturer requirement.
  • Inspect wheels, rims, hubs, axle areas, fasteners, bearings, and visible steering components for looseness, damage, or missing parts.
  • Inspect the underside for hydraulic fluid, oil, coolant, fuel, or other leaks and for loose, bent, or dragging components.
  • Check stabilizers, outriggers, support wheels, or anti-tip devices where fitted for damage and secure condition.
  • Escalate severely damaged tires, loose wheels, cracked rims, major leaks, missing fasteners, or undercarriage defects affecting stability or control.
Section 5Brakes, steering, travel controls, and hydraulics
  • Test service brakes for controlled stopping without abnormal delay, pulling, excessive travel, noise, or warning indications.
  • Verify the parking brake holds and releases correctly under the site's approved operational check.
  • Check steering response, steering wheel or tiller condition, directional stability, and any abnormal free play or steering effort.
  • Test accelerator or traction control, forward and reverse selection, inching or creep control where fitted, and neutral or interlock behavior.
  • Test lift, lower, tilt, reach, side-shift, attachment, and other hydraulic controls for smooth, predictable response without uncontrolled drift.
  • Remove the forklift from service when braking, steering, travel, interlocks, or hydraulic controls do not respond safely.
Section 7Battery, charging, fuel, and energy systems
  • Inspect battery case, retainers, cables, terminals, connectors, covers, plugs, and visible wiring for damage, looseness, corrosion, or overheating.
  • Check state of charge or battery indication is adequate for the planned task and no battery warning or fault is active.
  • Inspect LPG cylinder, tank, hose, fittings, valve, connector, and retaining system for secure condition and visible leak clues where applicable.
  • For diesel, gasoline, or other fueled forklifts, inspect fuel tank, cap, lines, fittings, and visible fuel leaks under the approved procedure.
  • Check charging or fueling connections are disconnected and secured correctly before the forklift enters operation.
  • Remove the forklift from service for battery damage, exposed conductors, overheating, fuel or LPG leaks, insecure cylinders, or other critical energy-system defects.
Section 9Planned task, route, load, and work-area readiness
  • Confirm the planned load weight, dimensions, stability, pallet condition, and center of gravity are suitable for the forklift and attachment.
  • Check the intended route, aisle, rack, dock, trailer, ramp, floor, or yard area is compatible with the forklift type and condition.
  • Verify floor surfaces, gradients, clearances, overhead obstructions, doorways, rack openings, and dock edges do not create an immediate equipment hazard.
  • Check pedestrian routes, crossings, blind corners, barriers, warning controls, and traffic rules are understood for the planned operating area.
  • Confirm special environments such as cold rooms, hazardous areas, outdoor yards, wet locations, or restricted zones use suitable approved equipment.
  • Escalate planned tasks that exceed capacity, use unsuitable attachments, require unsafe routes, or place the forklift in an environment it is not approved to enter.
Section 2Forks, carriage, mast, chains, and attachments
  • Inspect forks for cracks, bends, heel wear, damaged tips, unequal height, surface damage, or other visible defects.
  • Check fork retaining pins, locking devices, hooks, stops, positioners, and carriage mounting points are present and secure.
  • Inspect mast channels, uprights, rollers, chains, anchors, sheaves, welds, and visible fasteners for damage, wear, or looseness.
  • Check hydraulic hoses, cylinders, fittings, and connections around the mast and attachments for leakage, abrasion, or damage.
  • Inspect load backrest, side shift, fork positioner, clamps, rotators, platforms, booms, or other approved attachments for condition and secure mounting.
  • Remove the forklift from service when cracked forks, damaged mast components, broken chains, insecure attachments, or missing retaining devices could cause load loss.
Section 4Operator compartment and restraint systems
  • Inspect operator seat, seat mount, adjustment, seat belt or restraint, armrests, and presence-sensing systems where fitted.
  • Check overhead guard, cab, roof, side restraints, doors, gates, protective screens, and other operator-protection structures for visible damage.
  • Inspect steps, footplates, handholds, grab handles, floorboards, pedals, and access surfaces for secure and slip-resistant condition.
  • Check the operator compartment is free from loose objects, waste, fluids, or obstructions that could interfere with safe control use.
  • Verify dead-man controls, seat switches, gate interlocks, access controls, and operator-presence devices function where fitted.
  • Remove the forklift from service when restraints, operator-protection structures, access points, or presence controls are missing or unsafe.
Section 6Horn, alarms, lights, visibility, and labels
  • Test the horn and any required audible warning device for clear operation.
  • Check backup alarm, travel alarm, beacon, strobe, blue or red warning light, or pedestrian-alert system where fitted.
  • Inspect headlights, work lights, brake lights, direction indicators, mast lights, and other required lighting for condition and operation.
  • Check mirrors, cameras, sensors, windows, screens, and other visibility aids are clean, secure, and usable.
  • Verify capacity plates, data plates, warning labels, control labels, attachment information, and required operating markings are legible.
  • Escalate failed horns, alarms, required lights, visibility aids, or missing capacity information when they affect safe operation.
Section 8Operational response, unusual noise, and warning indications
  • Start the forklift using the approved procedure and observe the instrument panel for active warning lights, fault codes, or abnormal indications.
  • Listen for unusual engine, motor, hydraulic, mast, wheel, bearing, or drivetrain noise during the approved operational check.
  • Check for abnormal vibration, jerking, delayed response, uncontrolled movement, pulling, instability, or unusual steering behavior.
  • Verify lift, tilt, reach, attachment, and travel functions operate without leaks, sticking, excessive drift, or erratic response.
  • Check the forklift can stop, park, and remain stable under the approved pre-use test before entering normal service.
  • Remove the forklift from service when warning indications, noise, vibration, drift, or control behavior suggests an unresolved safety defect.
Section 10Defects, isolation, repair, and release
  • Classify findings by forks or mast, tires or wheels, operator protection, brakes, steering, controls, warnings, energy systems, task suitability, or other approved category.
  • Prioritize findings using load-drop potential, collision risk, loss of control, tip-over potential, fire or energy risk, recurrence, and planned task exposure.
  • Apply immediate controls such as removing the forklift from service, tagging or locking access, parking in the approved defect area, or restricting use.
  • Create corrective actions with named owners, due dates, repair scope, service-provider support, evidence expectations, and reinspection criteria.
  • Verify safety-critical repairs are complete and the relevant functions are reinspected before the out-of-service status is removed.
  • Record final pre-use status, critical open defects, restrictions, repair reference, inspector or operator, maintenance owner, warehouse owner, and release approval.
