Section 1Audit setup, hazard assessment, and PPE governance
- Confirm the project or site, review date, active work phase, inspector, site manager, PPE coordinator, and escalation contacts.
- Define the work zones, trades, contractors, tasks, plant interfaces, chemicals, noise sources, height work, and other exposures included in the review.
- Review current risk assessments, JHAs or task plans and verify they identify the PPE required after higher-level controls have been considered.
- Verify the current site PPE matrix, client rules, task-specific requirements, and local regulatory requirements are available to supervisors and workers.
- Confirm responsibilities are assigned for PPE approval, issue, inspection, cleaning, storage, replacement, respiratory protection, fall protection, and corrective action.
- Capture the review start time, geo-location, active site conditions, and an approved reference photo of the PPE compliance area.
Section 3Head protection and high-visibility workwear
- Verify workers exposed to impact, falling or flying objects, or relevant electrical hazards are wearing task-appropriate protective helmets.
- Check helmet shell, suspension, retention system, chin strap where required, adjustment points, and accessories for damage, contamination, unauthorized modification, or poor fit.
- Confirm head protection is the correct type or class for the site hazard and is used within the manufacturer's service and replacement guidance.
- Verify helmet-mounted lights, visors, earmuffs, face protection, and other accessories are approved or compatible and do not reduce the protection provided.
- Confirm workers exposed to vehicle, mobile-plant, traffic, or low-visibility interfaces wear the required high-visibility clothing in serviceable condition.
- Observe whether head and high-visibility PPE remains correctly worn during bending, climbing, lifting, hot work, equipment operation, and other normal task movements.
Section 5Hand, arm, and protective clothing compliance
- Verify gloves are selected for the actual cut, puncture, abrasion, heat, cold, chemical, biological, vibration, or other hand hazard present.
- Confirm glove size and fit support safe grip and dexterity without excessive looseness, bunching, exposed skin, or interference with tools and controls.
- Inspect gloves, sleeves, aprons, coveralls, chemical garments, and other protective clothing for tears, holes, damaged coatings, contamination, failed closures, or wear.
- Verify protective clothing provides the required coverage for sparks, hot work, chemicals, dust, weather, contamination, or other task-specific exposures.
- Check sleeves, cuffs, loose garments, and glove use do not create entanglement or caught-in hazards around rotating or moving equipment.
- Confirm contaminated or damaged hand and body PPE is removed from service, cleaned or disposed of correctly, and replaced before the worker is re-exposed.
Section 7Hearing protection compliance
- Verify current noise assessments or site rules identify the work areas and tasks where hearing protection is required.
- Observe sampled workers in designated noise areas and confirm required earplugs, earmuffs, or other approved protectors are worn throughout the exposure period.
- Confirm hearing protectors fit correctly and provide appropriate protection for the task without unsafe gaps, poor insertion, loose seals, or interference from other PPE.
- Check helmets, eyewear, respirators, hair, clothing, or communication equipment do not break earmuff seals or otherwise reduce hearing-protection effectiveness.
- Inspect reusable protectors for damaged cushions, headbands, plugs, hygiene issues, contamination, or deterioration and replace defective items promptly.
- Verify workers know when hearing protection is mandatory, how to fit it correctly, how to care for it, and when to request replacement or a different type.
Section 9Fall protection and task-specific specialty PPE
- Verify workers using personal fall protection have the correct full-body harness, connector, lanyard, self-retracting device, or other approved component for the system and task.
- Confirm fall-protection PPE is properly fitted, adjusted, connected, and compatible with anchors, connectors, lifelines, and the planned work method.
- Inspect fall-protection equipment before use and review required periodic inspections, labels, service status, impact history, damage, contamination, and removal-from-service criteria.
- Verify task-specific PPE such as welding shields, flame-resistant or arc-rated clothing, chemical suits, flotation equipment, or specialist protection is provided where the hazard assessment requires it.
- Confirm specialty PPE does not conflict with core PPE such as helmets, eye protection, respirators, gloves, footwear, or high-visibility garments.
- Where personal fall arrest or other specialized PPE is relied upon, verify emergency or rescue arrangements are available and understood by the responsible team.
Section 2PPE selection, proper fit, compatibility, and approval
- Verify each sampled PPE item is selected for the specific hazard, task, exposure level, and working environment it is intended to control.
- Confirm PPE is selected to properly fit each affected worker without unsafe looseness, restriction, pressure points, gaps, or reduced coverage.
- Check multiple PPE items can be worn together without breaking seals, dislodging helmets, obstructing vision, reducing hearing protection, or creating entanglement hazards.
- Verify required product markings, ratings, approvals, user instructions, and limitations are legible and appropriate for the intended use.
- Confirm workers who use personally owned PPE are subject to the same adequacy, condition, fit, maintenance, and sanitation checks as company-issued PPE.
- Review reported fit problems, unavailable sizes, discomfort, repeated adjustment, or non-use and confirm suitable alternatives are provided before exposure continues.
Section 4Eye and face protection
- Verify appropriate eye or face protection is worn for flying particles, dust, splashes, chemicals, grinding, cutting, welding, or other identified eye and face hazards.
- Confirm eye protection provides side protection where flying-object exposure requires it and remains secure during the task.
- Check prescription users have compliant protective eyewear or over-spectacle protection that does not disturb the position or effectiveness of either lens system.
- Verify welding, cutting, brazing, and other optical-radiation tasks use the correct filter or shade and suitable face protection for the exposure.
- Inspect lenses, frames, seals, straps, visors, face shields, and adjustment points for cracks, deep scratches, contamination, clouding, distortion, or poor fit.
- Confirm reusable eye and face PPE is cleaned, disinfected where needed, stored to prevent damage, and replaced when vision or protection is compromised.
Section 6Foot protection and electrical protective equipment
- Verify protective footwear is required and worn where workers are exposed to falling or rolling objects, puncture, crushing, sharp debris, or other identified foot hazards.
- Inspect footwear fit, uppers, toe protection, soles, tread, laces or closures, penetration resistance, and contamination for conditions that could reduce protection or create slips.
- Confirm metatarsal, chemical-resistant, waterproof, heat-resistant, or other specialized foot protection is provided where the task-specific assessment requires it.
- Where electrical work requires electrical protective equipment, verify gloves, sleeves, blankets, covers, or related items are selected for the hazard and used with compatible protectors.
- Check electrical protective equipment inspection, test status, markings, storage, and removal-from-service controls meet the applicable site and regulatory requirements.
- Observe whether mud, standing water, concrete contamination, worn tread, damaged footwear, or unsuitable sole design is increasing slip, trip, or electrical risk.
Section 8Respiratory protection compliance
- Verify respiratory hazards are evaluated and the selected respirator, filter, cartridge, or supplied-air system is appropriate for the contaminant and work conditions.
- Confirm workers required to use respirators are covered by the applicable respiratory-protection program, including required medical evaluation and training.
- Verify required fit testing is current for workers using tight-fitting respirators and matches the make, model, style, and size being used.
- Observe a user seal check and confirm facial hair, eyewear, headgear, or other conditions do not interfere with the sealing surface or valve function of tight-fitting respirators.
- Check filters and cartridges are correct, within service life, properly installed, protected from contamination, and changed under the approved schedule or end-of-service criteria.
- Inspect respirators for cleanliness, facepiece damage, straps, valves, connections, storage condition, and any defect that requires removal from service.
Section 10PPE use, care, replacement, training, actions, and sign-off
- Observe a representative worker sample and calculate PPE compliance by task, contractor, work zone, and PPE category, highlighting every critical gap.
- Verify workers wear required PPE correctly for the full exposure period and do not remove, modify, share, or bypass equipment in a way that reduces protection.
- Review PPE issue, inspection, cleaning, storage, fit-testing, training, replacement, and stock records for completeness, traceability, and overdue requirements.
- Confirm workers and supervisors can explain PPE purpose, limitations, correct donning and doffing, fit, inspection, care, defect reporting, and replacement rules.
- Create corrective action for missing, unsuitable, ill-fitting, damaged, contaminated, expired, incorrectly used, or unavailable PPE and verify immediate exposure controls.
- Record the final compliance result, remaining work restrictions, owners, due dates, follow-up requirements, reviewer approval, date, time, and signature.
