  • Confirm warehouse or site, equipment type, fleet or asset ID, manufacturer or model where used, inspection date, shift, and inspection type are clearly identified.
  • Record hour-meter reading where applicable, assigned operating zone, current attachment or configuration, operator or inspector, fleet owner, and maintenance owner.
  • Confirm the operator is trained, evaluated, and authorized for the specific type of powered industrial truck or equipment they will operate.
  • Review previous pre-use defects, maintenance work orders, breakdowns, impact events, battery or fuel issues, attachment problems, and unresolved repair items.
  • Confirm the equipment is the correct type and capacity for the planned warehouse task, load, aisle, rack height, dock, trailer, floor, and environmental condition.
  • Record the fleet's final inspection status such as serviceable, restricted use, repair before use, or out of service.
Section 3Brakes, parking brake, steering, accelerator, direction control, lift and tilt controls, hydraulics, and operational response
  • Check service brakes provide controlled stopping without abnormal travel, pulling, delay, noise, or warning indication under the site's approved operational test.
  • Verify the parking brake holds and releases correctly and shows no obvious control, cable, hydraulic, or mechanical defect.
  • Check steering response, steering wheel or tiller condition, free-play clues, power-steering response, and directional stability during the approved operational check.
  • Test accelerator or traction control, forward and reverse direction control, inching or creep control where fitted, and neutral or interlock behavior for correct response.
  • Test lift, lower, tilt, side-shift, attachment, reach, clamp, platform, or other operating controls for smooth, predictable response without drift or uncontrolled movement.
  • Escalate ineffective brakes, failed parking brake, steering defects, uncontrolled hydraulic movement, failed interlocks, or other operational-control faults that make the equipment unsafe.
Section 5Operator compartment, seat and restraint, overhead guard, steps, handles, pedals, platform, gates, and protective structures
  • Inspect operator seat, seat mount, adjustment, seat belt or operator restraint, armrests, presence-sensing devices, and seat switches where fitted.
  • Check overhead guard, load guard, protective screen, cab, roof, door, side restraint, operator enclosure, or other protective structure for visible damage or unauthorized modification.
  • Inspect steps, footplates, handholds, grab handles, entry surfaces, floorboards, pedals, mats, and operator-compartment housekeeping for secure and slip-resistant condition.
  • For order-picking or elevating operator equipment, inspect platform rails, gates, chains, controls, harness or fall-protection interfaces where required, and platform floor condition.
  • Verify pedals, deadman controls, foot switches, operator-presence systems, interlocks, gates, and access controls function as intended during the approved operational test.
  • Escalate missing or damaged restraints, compromised overhead guards, failed presence controls, defective platform gates, unsafe operator access, or other protective-structure defects.
Section 7Battery, charging, electrical condition, LPG, diesel or fuel systems, leaks, connectors, and energy source safety
  • Inspect battery condition, case, hold-downs, cables, terminals, connectors, covers, plugs, interlocks, and visible wiring for damage, looseness, corrosion, or overheating clues.
  • Check battery charge level or state-of-charge indication, charger compatibility, connector condition, charging leads, cable routing, and charger mounting where applicable.
  • Inspect battery-changing or charging areas for ventilation, housekeeping, spill response, eyewash or emergency resources where required, ignition control, and suitable charging procedures.
  • For LPG equipment, inspect cylinder mounting, retaining devices, hoses, fittings, valves, connectors, protective guards, and visible leak or damage clues.
  • For diesel, gasoline, or other fueled equipment, inspect fuel tank, cap, lines, fittings, leaks, exhaust condition, and fueling controls under the approved programme.
  • Escalate battery damage, exposed conductors, overheating, fuel or LPG leaks, damaged connectors, insecure cylinders, charger faults, or other energy-source conditions creating immediate risk.
Section 9Dock equipment, mobile ramps, lift tables, scissor lifts, conveyors, hoists, and adjacent material-handling equipment
  • Inspect dock levelers, dockboards, bridge plates, mobile ramps, vehicle restraints, lift tables, scissor lifts, or similar dock equipment for visible structural or operational defects.
  • Check conveyor guards, emergency stops, pull cords, covers, rollers, belts, chains, sensors, and access points for visible damage or bypass where conveyors are included in scope.
  • Inspect hoists, winches, lifting tables, pallet positioners, work platforms, and other handling devices for visible damage, controls, guards, limit devices, and service status within inspector authorization.
  • Verify equipment capacity labels, operating limits, manufacturer markings, inspection status, or service tags are legible where required by the site programme.
  • Check surrounding floors, charging or parking areas, access routes, barriers, housekeeping, and storage do not obstruct safe equipment operation or emergency controls.
  • Escalate damaged lifting structures, failed guards, bypassed emergency stops, defective dock equipment, or other material-handling equipment conditions presenting immediate serious risk.
Section 2Forks, mast, carriage, chains, rollers, load backrest, attachments, retaining devices, and structural condition
  • Inspect forks for cracks, bends, tip damage, heel wear, unequal height, surface damage, or other visible defects affecting safe load engagement.
  • Check fork retaining pins, locking devices, hooks, positioners, fork stops, and carriage mounting points are present, secure, and correctly engaged.
  • Inspect mast channels, uprights, rollers, chains, anchors, sheaves, hoses, cylinders, welds, and visible fasteners for damage, looseness, leakage, or abnormal wear.
  • Check load backrest extension, carriage, side-shift, fork-positioner, clamps, rotators, booms, platforms, or other approved attachments for visible condition and secure mounting.
  • Verify attachment identification, rated capacity information, controls, hydraulic connections, and truck data plate or capacity information reflect the approved configuration where applicable.
  • Escalate cracked or bent forks, damaged mast structure, broken chains, insecure attachments, missing retaining devices, or other load-handling defects that could cause load drop or collapse.
Section 4Tires, wheels, casters, axles, hubs, steering components, suspension, undercarriage, and floor-contact condition
  • Inspect pneumatic, solid, cushion, polyurethane, load, drive, steer, and caster wheels for cuts, chunking, flat spots, embedded objects, severe wear, or separation.
  • Check tire inflation where pneumatic tires are used and compare visible condition with the approved manufacturer or fleet requirement.
  • Inspect wheels, rims, hubs, axle areas, wheel fasteners, bearings, retaining devices, and visible steering components for looseness, damage, leakage, or missing parts.
  • Check pallet-truck load wheels, rollers, stabilizers, outriggers, reach-truck support wheels, and order-picker running gear for cracks, wear, flat spots, or binding.
  • Inspect the underside for fluid leaks, loose components, dragging parts, damaged guards, bent brackets, debris buildup, or impact evidence.
  • Escalate severely damaged tires, loose wheels, missing fasteners, cracked rims, failed casters, damaged axles, or undercarriage defects that affect equipment stability or control.
Section 6Horn, alarms, lights, mirrors, cameras, warning devices, indicators, gauges, data plate, and visibility
  • Test horn and any required audible warning device for clear operation and suitable control access.
  • Check backup alarm, travel alarm, blue or red warning light, beacon, strobe, pedestrian-alert system, or other site-required warning devices where fitted.
  • Inspect headlights, work lights, rear lights, brake lights, direction indicators, mast lights, platform lights, or task lighting where equipped and required.
  • Check mirrors, cameras, proximity systems, sensors, displays, and visibility aids for cleanliness, alignment, image quality, damage, and correct operation where fitted.
  • Review hour meter, battery or fuel gauge, temperature, oil, hydraulic, brake, fault, service, or other warning indicators for abnormal readings or active faults.
  • Verify data plate, capacity information, attachment information, warning labels, operating decals, and critical safety markings are legible and appropriate to the equipment configuration.
Section 8Pallet trucks, reach trucks, stackers, order pickers, tow tractors, mobile equipment, and task-specific checks
  • Inspect powered pallet trucks for tiller condition, emergency reverse device, travel controls, lift and lower function, load wheels, drive wheel, forks, battery, horn, and visible damage.
  • Inspect reach trucks for reach mechanism, pantograph or moving mast components, outriggers, support wheels, forks, mast, controls, visibility, and warning systems.
  • Inspect stackers for mast, forks, support legs, controls, parking function, wheels, steering or tiller, load-handling condition, and operator-protection features.
  • Inspect order pickers for operator platform, gates, controls, mast, forks or load platform, fall-protection interfaces where required, lowering system, alarms, and emergency controls.
  • Inspect tow tractors, tuggers, carts, dollies, coupling devices, drawbars, pins, safety chains where used, brakes, steering, wheels, and load-train condition.
  • Escalate equipment-specific defects that affect safe travel, lifting, operator protection, load support, coupling, emergency lowering, or other critical task function.
Section 10Defect reporting, out-of-service control, lockout/tagout, repair, reinspection, recurring trends, and final sign-off
  • Classify findings by forks or mast, brakes or steering, tires or wheels, controls, operator protection, warning devices, battery or fuel, attachment, equipment-specific component, dock equipment, or other approved category.
  • Prioritize defects by potential for dropped load, loss of control, collision, tip-over, operator injury, fire, electrical exposure, equipment collapse, recurrence, and urgency.
  • Remove powered industrial trucks or other equipment from service when defects adversely affect safe operation, and clearly identify the unit so it is not used pending correction.
  • Where servicing or maintenance could expose workers to unexpected energization, startup, movement, hydraulic pressure, gravity, electrical, or other hazardous energy, apply the controlling lockout/tagout or energy-control procedure.
  • Verify repairs are completed by authorized personnel, replacement parts and modifications maintain the approved safety configuration, and significant repairs are reinspected before return to service.
  • Record final equipment status, out-of-service controls, open defects, work-order reference, repair evidence, next inspection or service date, inspector, operator where applicable, maintenance owner, and release approval.

Does lockout/tagout apply during forklift or equipment maintenance?
