Section 1Inspection setup, PPE inventory, suitability, and fit
  • Confirm the project, work area, inspection date, inspector, responsible supervisor, contractor, affected work groups, and approver.
  • Define the tasks, hazards, trades, work zones, and PPE categories included in the inspection.
  • Verify required PPE has been selected for the actual hazards and is of safe design and construction for the work being performed.
  • Confirm sampled PPE properly fits each affected worker and does not create gaps, restriction, snagging, instability, or other fit-related hazards.
  • Review previous PPE defects, damaged items, issue records, replacements, reported discomfort, near misses, and overdue corrective actions.
  • Capture the inspection start time, work location, and approved reference photos of representative PPE and active work conditions.
Section 3Eye and face protection inspection
  • Inspect safety glasses, goggles, face shields, welding shields, and lenses for cracks, deep scratches, pitting, clouding, distortion, heat damage, or chemical attack.
  • Check frames, temples, straps, hinges, side protection, retainers, headgear, and face-shield mounts for secure condition and reliable adjustment.
  • Verify the selected eye or face protector matches the actual hazard, including flying particles, splash, dust, molten material, chemical exposure, or injurious light radiation as applicable.
  • Confirm eye protection fits snugly without unduly interfering with movement and remains correctly positioned during the worker's normal task.
  • Verify prescription eyewear arrangements do not displace required protective lenses and that side protection is present where flying-object hazards require it.
  • Remove dirty, damaged, poorly fitting, modified, or optically distorted protectors from use until cleaned, repaired, or replaced.
Section 5Hand, arm, and glove inspection
  • Inspect work gloves for cuts, punctures, tears, abrasion, worn fingertips, split seams, damaged coatings, chemical attack, burns, stiffness, or loss of grip.
  • Verify glove material and construction are suitable for the specific task hazard, such as cuts, abrasion, chemicals, heat, cold, vibration, or electrical exposure where applicable.
  • Check gloves fit the worker correctly and do not create excess loose material, restricted dexterity, exposed skin, or snagging risk around tools and equipment.
  • Inspect gauntlets, cuffs, sleeves, arm guards, and related hand or arm protection for secure coverage and compatibility with adjacent PPE.
  • Confirm reusable gloves are cleaned, dried, stored, and segregated as needed to prevent contamination, degradation, or unintended reuse for incompatible tasks.
  • Remove from service gloves with unknown contamination, damaged protective layers, failed seams, degraded material, or other defects that could compromise protection.
Section 7Respiratory protective equipment inspection
  • Verify each respirator is the correct type and configuration for the assigned task, contaminant, work condition, and respiratory protection program requirements.
  • Inspect facepieces for cracks, tears, distortion, dirt, residue, damaged sealing surfaces, deteriorated elastomer, or missing components.
  • Check head straps, harnesses, valves, valve covers, gaskets, cartridges, filters, canisters, connectors, hoses, and other components for condition and correct assembly.
  • Confirm filters, cartridges, and canisters are the correct type, within required service or change-out limits, properly seated, and protected from contamination before use.
  • Verify tight-fitting respirators are not used when facial hair, eyewear, headgear, damaged seals, or other conditions interfere with the face-to-facepiece seal.
  • Confirm reusable respirators are cleaned, disinfected, inspected, dried, and stored to protect them from damage, contamination, dust, sunlight, extreme temperature, moisture, and damaging chemicals.
Section 9Electrical and specialty PPE inspection
  • Inspect electrical protective gloves, sleeves, blankets, covers, mats, face protection, arc-rated clothing, and other specialty PPE used for the inspected electrical task.
  • Check rubber insulating equipment for cuts, punctures, cracking, ozone damage, embedded foreign material, swelling, chemical damage, contamination, or other physical defects.
  • Verify required electrical PPE markings, class, rating, test status, and identification are legible and appropriate for the exposure being controlled.
  • Confirm arc-rated or flame-resistant garments are clean, undamaged, properly fastened, correctly layered where required, and not contaminated with substances that could increase burn risk.
  • Inspect chemical splash, welding, abrasive-blasting, chainsaw, flotation, and other specialty PPE for task-specific damage, missing parts, expired components, or loss of protective function where applicable.
  • Remove any specialty PPE from service when inspection, testing, rating, contamination, damage, or manufacturer criteria indicate it is not suitable for continued use.
Section 2Head protection inspection
  • Inspect protective helmets or hard hats for cracks, penetration, deformation, brittleness, deep cuts, impact marks, chemical damage, heat damage, or other shell deterioration.
  • Check suspension systems, straps, adjustment mechanisms, attachment points, chin straps where used, and internal components for wear, damage, contamination, or missing parts.
  • Verify the helmet type and electrical protection class, where applicable, are suitable for the hazards present in the inspected work area.
  • Confirm helmets fit securely, are adjusted to the worker, remain stable during normal movement, and are not altered in a way that could reduce protection.
  • Check manufacturer markings or identification remain legible enough to confirm the product and required protective characteristics.
  • Remove from service any helmet that has sustained significant impact, shows material degradation, has unauthorized modifications, or cannot be maintained in reliable condition.
Section 4Hearing protection inspection
  • Inspect earplugs, earmuffs, helmet-mounted hearing protectors, and reusable hearing devices for damage, contamination, deformation, hardened material, or missing components.
  • Check earmuff cups, seals, cushions, headbands, pivots, helmet adapters, and tension for wear that could reduce the acoustic seal.
  • Confirm disposable earplugs are clean, unused where single-use rules apply, and stored to prevent contamination before use.
  • Verify reusable plugs are clean, pliable, undamaged, and suitable for the worker's ear canal and the required fitting method.
  • Confirm workers can achieve the intended fit without helmets, eyewear, hair, clothing, or other PPE breaking the hearing-protector seal.
  • Replace hearing protectors that are damaged, contaminated, no longer fit correctly, or cannot be maintained in a sanitary and reliable condition.
Section 6Foot, leg, body, and high-visibility PPE inspection
  • Inspect safety footwear for damaged toe protection, worn or detached soles, punctures, splits, exposed components, poor tread, loose stitching, or other defects affecting protection.
  • Check footwear fit, lacing or fastening, ankle support where required, slip resistance, sole condition, and compatibility with the actual work surface and task.
  • Inspect high-visibility garments for fading, contamination, torn fabric, damaged fasteners, missing reflective material, or reduced conspicuity in the work environment.
  • Check coveralls, protective clothing, aprons, chemical garments, knee protection, and leg protection for tears, holes, worn areas, contamination, failed closures, or insufficient coverage.
  • Confirm protective garments fit properly and do not create excessive loose material, restricted movement, trip hazards, exposed skin, or interference with harnesses and other PPE.
  • Remove footwear or protective clothing from service when damage, contamination, wear, poor fit, or loss of protective properties makes continued use unreliable.
Section 8Fall protection PPE inspection
  • Inspect full-body harness webbing for cuts, tears, abrasion, burns, chemical damage, excessive stretching, broken fibers, mold, contamination, or other deterioration.
  • Check D-rings, buckles, adjusters, stitching, labels, attachment points, connectors, and hardware for distortion, cracks, corrosion, sharp edges, loose components, or impaired function.
  • Inspect lanyards, self-retracting devices, lifelines, rope grabs, energy absorbers, and connectors for wear, cuts, damaged housings, broken stitching, impact indicators, corrosion, or malfunction.
  • Verify locking snaphooks and carabiners close and lock correctly and are compatible with the intended anchorage or connecting component.
  • Confirm personal fall-arrest and positioning components are inspected before each use and any component showing wear, damage, deterioration, or other significant defect is removed from service.
  • Immediately remove from service any fall-arrest component subjected to impact loading until it has been inspected and determined by a competent person to be undamaged and suitable for reuse.
Section 10Cleaning, storage, replacement, records, and sign-off
  • Verify reusable PPE is cleaned and, where required, disinfected using methods compatible with the equipment and manufacturer instructions.
  • Check PPE storage protects equipment from crushing, sunlight, moisture, chemicals, dirt, heat, sharp objects, deformation, and unauthorized use.
  • Confirm defective, contaminated, expired, failed, or withdrawn PPE is clearly identified and segregated so it cannot return to service unintentionally.
  • Review issue, inspection, maintenance, test, cleaning, replacement, and disposal records for PPE categories where site procedures or equipment requirements call for traceability.
  • Assign each PPE defect or inspection finding an owner, priority, due date, replacement or corrective action, escalation requirement, and closure evidence rule.
  • Record the final PPE inspection result, critical defects, items removed from service, replacements required, remaining restrictions, next review date, inspector, supervisor, approver, date, time, and sign-off.
